Request a Copy of Your Exam Images & Report

Please call the location you visited to have your exam images and report copied and prepared for you, or to have them sent to another facility or physician.

Your exam images and reports are confidential and will require you to provide identification before they can be mailed, faxed or handed to you.

You are the only one allowed to receive your own images unless you authorize someone else to do so. You may authorize another person to pick up your exam images and report, by filling out an authorization form. This written authorization must be signed by you and must include the name of the person authorized to receive your images and report.

Your images are the property of MemorialCare. We are legally required to maintain them for seven years. If you received film images you are required to return your film images within 30 days of check out.
