The Congestive Heart Failure Clinic provides multidisciplinary management for congestive heart failure patients in an outpatient setting. Rehabilitation at the Congestive Heart Failure Clinic is conducted by a physician and RNs.The goal of Congestive Heart Failure rehabilitation is to reduce hospital readmissions and improve quality of life.
Congestive Heart Failure rehabilitation includes:
- Education using videos, written material and one-on-one teaching.
- Pharmaceutical, dietary and social services counseling.
- Physician consultations.
- Supervised exercise.
